• Coordinate and or assist with participant ADL care as necessary in the center and in the participant's home.
  • Coordinate and or assist with participant meals and fluid intake on a daily basis to assure proper nutrition is maintained as defined by participant care plan.
  • Consistently assess each assigned participant's condition through professional clinical processes, critical thinking, and judgment, to meet the needs of participant in manner to avert avoidable changes in condition.
  • Notify Director of Clinical/Home Care Services or RN when participant condition changes and when an issue arises with other staff members.
  • Administer and document medications and treatments in accordance with established policies and procedures as indicated.
  • As requested, will check the auto medication order for accuracy.
  • Will communicate with pharmacy as needed.
  • Assure that all necessary medications/supplies/paperwork are sent home with participants on a daily basis.
  • Participate in team meetings, committee meetings, staff meetings, family meetings, etc. as requested.
  • Update participant care plans as needed.
  • Assist in completion of documentation as related to participant care on a daily or as necessary/scheduled basis (e. g., chart documentation, MAR).
  • Assist physician as needed in examination room or other areas of clinic.
  • Transcribe physician orders per policy in a timely manner.
  • Observe nursing care and visit participants daily to assure that nursing care is carried out as directed by the care planning process.
  • Participate in giving and receiving daily report to assure continuity of care.
  • Communicate necessary information to Program Assistants.
  • Complete participant incident/accident reports, per policy in a timely manner.
  • Coordinate and/or participates in the daily collecting of vitals and bodily samples such as urine, sputum and blood sugars, including lab draws from participant for testing, as needed.
  • Communicate abnormal lab results to appropriate physicians in a timely manner as indicated per policy.
  • Interact and communicate information as necessary to families, POA's, participants, staff, etc., maintaining HIPPA guidelines.
  • Assure that appropriate and adequate supplies are available in clinic and homes for all disciplines.
  • Facilitate and participate in team meetings as requested to provide and enhance communication between all disciplines.
  • Facilitate and participate in problem resolution regarding employees and other clinic concerns.
  • Participate in orientation and training of personnel.
  • Assure a clean, safe and orderly environment for participants, staff and visitors.
  • Participate in assuring that accurate documentation of participant care is provided during and before end of the shift.
  • Works with the Director of Clinical/Home Care Services and RN to oversee the provision of ancillary and specialty services.
  • Communicates with ancillary providers including dentist, podiatrist, pharmacist, audiologist and ophthalmologist as requested in regard to the care of the participants.
  • Communicates with ancillary and specialty services as requested to share pertinent participant information.
  • Provide input to Director of Clinical/Home Care Services or RN concerning nurse and nurse aide performance.
  • Maintain confidentiality according to HIPAA standards and protocols.
  • Participate in the on-call rotation.
  • Attend and participate in internal and external meetings, in-services, and continuing education programs.
  • Must maintain eight hours of in-service time per year.
  • Performs other duties and responsibilities as assigned.


  • Must be a graduate of an accredited nursing program.
  • Two years nursing experience. Preferably in home care. Must have one year experience with the frail or elderly population.
  • Working knowledge of computers, Microsoft Word and Excel required.
  • Must have dependable transportation with a valid driver's license in order to visit clients in their homes and other sites.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
