Physician

Responsibilities:

  • Completes a comprehensive history and physical (H&P) on new referrals.
  • Completes a comprehensive H&P three months after enrollment. Primary purpose is for diagnosis data entry into the Risk Assessment Process System (RAPS)
  • Completes a semi-annual (H&P) on established participants.
  • Evaluation and treatment of participants during acute illness.
  • Determining the need and facilitating referrals to other medical specialists when deemed necessary.
  • Discussing care guidelines with participants.
  • Judicious utilization of specialist consultations.
  • Rotating night and weekend call with other LIFE primary care providers.
  • Is knowledgeable about participant s overall need and views the participant in a holistic manner.
  • Participates in the development and revision of the participant s plan of care as a member of the interdisciplinary team.
  • Integrates the primary care treatment plan into the overall plan of care developed by the interdisciplinary team.
  • Brings to the IDT essential information including Present and Advance Directives, Goals of Care prognostic information and other pertinent information as outlined in the policy and procedures for Care Planning and IDT Meetings.
  • Interacts with other team members to meet emergent and acute needs of participants.
  • Documents changes in a participant s condition in the medical record according to policy and procedures.
  • Collaborates with nurse practitioner and other physicians to ensure comprehensive and continuous care.
  • Conducts home visits, hospital visits, and nursing visits as needed. Follows and coordinates all aspects of care.
  • Maintains quality documentation of all services provided.
  • Maintains daily or as often as deemed necessary contact with the Hospitalists about the care of hospitalized participants coordinating with them their care and discharge planning.
  • Knows and adheres to the philosophy and goals of the LIFE Programs.
  • Maintains a safe environment for participants, visitors and staff.
  • Keeps confidentiality of participant records, reports and discussions.
  • Participates in formulation and maintenance of LIFE Program policies and procedures.
  • Participates in LIFE Committees as requested by the Medical Director and Executive Director. These include but are not limited to the following:
    • Polypharmacy Committee Meetings
    • Participant Care Committee
    • Quality Improvement Committee
    • Medical Clinic Meeting
    • Ethics Committee
  • Advises the Medical Director in ways and means to establish better accountability of LIFE services to clients and referral sources.
  • Attends and participates in scheduled staff meetings as requested.
  • Maintains flexibility in schedule and responds to unexpected emergencies and changes in workload in order to fulfill responsibilities.
  • Utilizes supplies and equipment economically.
  • Informs the Medical Director of unusual incidents.
  • Identifies service delivery problems and uses good judgment in their solution.
  • Provides in-service training as needed.
  • Interprets LIFE program to interested individuals & groups.
  • Is professional in appearance and manner in the clinical area; recognizes own limits and seeks help and guidance from the Medical Director as appropriate; responds in a positive manner to constructive criticism; serves as a role model for students and staff members.
  • Optionally holds membership in professional organizations.
  • Maintains licensure and certification and pursues professional growth through continuing education process.
  • Takes a leadership role in the medical clinic and in the coordination of LIFE medical staff.
  • Performs other related duties as required.
  • Admitting/practice privileges at appropriate area hospitals contracted with LIFE Programs as dictated by the Medical Director.
  • Performs other duties and responsibilities as assigned.

Qualifications

  • Physician must possess current M. D. or D. O. license from the state of Pennsylvania and a graduate of an accredited school of medicine.
  • Have at least one year experience with a frail or elderly population;
  • Experience in internal or geriatric medicine in a managed care environment preferred;
  • Physician must have working knowledge of current community health practice for the frail elderly from direct service experience, and have a working knowledge of the interdisciplinary model of care management;
  • Physician must have working with cognitively impaired seniors, and must be able to relate well with seniors and their families to deal with sensitive issues and facilitate problem solving;
  • Working knowledge of computers, Microsoft Word and Excel required.


Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
